Job summary

Atrinity Home Health, part of the Optum family, is seeking an Occupational Therapist to join our home health team. You will assess and evaluate patient care needs related to daily living, functional status, and home safety, and coordinate with the care team to implement plans of care.

This role emphasizes independence, strong communication, and driving quality outcomes while delivering compassionate care in patients' homes across Connecticut.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws governing Occupational Therapy, per plan of care.
• Evaluates functional status and CMS documentation needs; consults with physician on therapy plan.
• Observes and reports patient response to treatment to nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using standard tests.
• Function in home settings regardless of age, race, creed, color, sex, disability, or financial condition.
