Eligible for a $10,000 first-year success bonus for achieving key milestones. Travel to patient homes is required, with ongoing support from a dedicated field education team and leadership that encourages growth.

Qualifications

• Active Occupational Therapist, OT license in the state you work.
• One year of experience as an Occupational Therapist.
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
