Job summary

Residential Home Health and Hospice seeks an Occupational Therapist to provide skilled care in patients\' homes in Milford, PA. A $10,000 first-year bonus is available for achieving key milestones.

You will evaluate and treat ADLs and home safety, educate patients and caregivers, and document care in the EMR. Travel to patient homes is required within the assigned territory, with strong interdisciplinary support.

Qualifications

• Active Occupational Therapist license in the state you work.
• Minimum 1 year of Occupational Therapy experience.
• Current driver’s license and ability to travel ~20% of the day to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform comprehensive patient evaluations focusing on ADLs, upper extremity function, and home safety.
• Educate and empower patients and caregivers on adaptive techniques and home safety strategies.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
