Residential Home Health and Hospice is hiring an Occupational Therapist to provide skilled OT in patients' homes in the Jacksonville and Springfield areas. You will assess ADLs, design individualized plans, teach safety techniques, and collaborate with a supportive team.

Qualifications

• Active Occupational Therapist license in the state you work.
• One year of experience as an OT delivering patient care.
• Valid driver's license and ability to travel ~20% of work time.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting in collaboration with an interdisciplinary team.
• Perform comprehensive evaluations focusing on ADLs, upper extremity function, and home safety.
• Develop individualized treatment plans to promote independence and quality of life.
• Educate patients and caregivers on adaptive techniques, equipment, energy conservation, and safety.
• Ensure accurate EMR documentation and regulatory compliance.
• Travel to patient homes within a defined territory and work independently as needed.

We are hiring an Occupational Therapist (OT) for a Home Health position providing skilled care to patients in their homes in the Jacksonville, Springfield, and surrounding areas. This position is eligible for $20,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $10,000 sign-on bonus paid within the first 18 months of employment! What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you


• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life


• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines


• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation


• Work independently, but never alone


• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work


• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care


• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
