Residential Home Health and Hospice is seeking an Occupational Therapist to provide skilled home health care in the Media, PA area. This role offers autonomy, meaningful patient relationships, and career growth, with a $10,000 first-year bonus based on achieving key milestones.

You will assess ADLs, safety, and independence, educate patients and caregivers, and document thoroughly in the EMR. A valid driver’s license is required for travel to patient homes across the territory.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
