Job summary

Residential Home Health and Hospice in Springfield, IL is seeking an Occupational Therapist for a home health role providing skilled care in patients' homes within an assigned territory.

The position offers a total bonus potential of $20,000, including a $10,000 first-year success bonus and a $10,000 sign-on paid within 18 months. Additional bonus opportunities extend with tenure and performance.

Qualifications

• Active Occupational Therapist license in the state you work.
• One year of experience as an Occupational Therapist.
• Valid driver’s license and ability to drive approximately 20% of the workday.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, collaborating with an interdisciplinary team.
• Perform comprehensive evaluations focused on ADLs, upper extremity function, and home safety; develop individualized treatment plans.
• Educate caregivers on adaptive techniques, assistive equipment, energy conservation, and home safety strategies to support independence.
• Ensure compliance with EMR documentation and thorough charting.
• Travel to patient homes within the assigned territory.

What You'll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
