AHN Healthcare@Home in the Erie area is hiring a licensed Occupational Therapist to deliver skilled, patient-centered home health care. You will assess ADLs, upper extremity function, and home safety, develop individualized treatment plans, and educate patients and caregivers. The role requires driving to patient homes, collaborating with an interdisciplinary team, and maintaining compliant EMR documentation. Qualifications

• Active Occupational Therapist with license in the state you work.
• Minimum one year of OT experience in home health or equivalent.
• Valid driver’s license required for travel to patient homes.

Responsabilités

• Deliver skilled, patient-centered care in a home health setting with interdisciplinary team support.

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations focused on activities of daily living (ADLs), upper extremity function, and home safety, developing individualized treatment plans to promote independence and quality of life
• Educate and empower patients and caregivers on adaptive techniques, use of assistive equipment, energy conservation, and home safety strategies to support safe, confident participation in daily routines
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation

OT Qualifications

• Active Occupational Therapist, OT license in the state you work
• One year of experience as an Occupational Therapist and the clinical expertise to deliver quality patient care

• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
