Job summary

Your Health is seeking a Nurse Practitioner to provide in-home patient care in the Charleston area. This full-time, salary-based role includes collaboration with the care team, evaluation of high-risk patients, care coordination, and comprehensive management to improve outcomes.

You will perform visits in the designated service area, with weekday hours (8 AM–5 PM) and the potential for a monthly bonus. A strong emphasis on HIPAA compliance and coordination with facilities, clinics, and families

Qualifications

• Licensed Nurse Practitioner (NP), PA, or Physician with valid license in state of employment.
• Experience in patient care, preferably home health, clinic, facility, telehealth, or specialty care setting.
• Strong communication and collaboration within an interdisciplinary team.
• Ability to evaluate high-risk patients and develop care plans.
• Commitment to patient-centered care and excellence in healthcare delivery.
• Strong organizational and time management skills; multitasking ability.
• Must be able to travel between offices, facilities, etc. and have a valid driver's license.

Responsibilities

• Evaluate high-risk patients with complex needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity and outcomes.
• Communicate with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches and others.
• Participate in interdisciplinary team meetings to discuss progress and plans.
• Foster effective communication and collaboration for seamless management.
• Meet staff efficiency expectations while delivering high-quality in-home/clinic care.
• Document care management activities accurately to support plans and compliance.
• Provide patients with education and resources related to conditions, treatment, and prevention.
• Offer guidance to patients and families to enhance understanding of options.
• Adhere to healthcare regulations including HIPAA and Medicare guidelines.
• Monitor quality measures and identify gaps for improvement.
• Support care across homes, facilities, telehealth, clinics, and specialties.
