Job summary

NHC HomeCare Low Country in North Charleston, SC is seeking an experienced RN to provide in-home nursing care. The role uses the nursing process to assess and plan patient care, ensuring each patient’s health goals are met in their home environment.

You will collaborate with an interdisciplinary team, educate patients and caregivers, and document ongoing needs to support optimal health outcomes in a culturally respectful manner.

Qualifications

• Active RN license in SC or Compact State.
• Minimum 1 year RN experience.
• Home care experience desirable.

Responsibilities

• Utilizes the nursing process to assess, plan, implement, and evaluate patient care.
• Assess physical, psychosocial, and environmental health factors to develop care plans.
• Collaborates with the interdisciplinary team to provide and coordinate care and rehabilitation.
