Talin Home Healthcare LLC is seeking multiple compassionate and dedicated Home Health Nurses to serve patients across Northern Virginia, including Arlington, Alexandria, and Fairfax. You will provide skilled nursing care in patients' homes, assess needs, and develop personalized care plans with the care team to ensure high-quality outcomes.

Active Virginia RN license, strong clinical assessment and patient education skills, and ability to travel NOVA with reliable transportation are required.

Qualifications

• Active VA RN license required.
• Previous home health or med-surg experience preferred.
• Willingness to travel throughout NOVA.
• Strong communication and patient education skills.

Responsabilités

• Provide skilled nursing care to patients in their homes across the Northern Virginia area.
• Conduct comprehensive patient assessments and develop individualized care plans.
• Administer medications, wound care, IV therapy, ostomy care as ordered.
• Monitor and document patient progress, vital signs, and changes in condition.
• Educate patients and families on disease management and home care techniques.
• Collaborate with physicians and care team members to ensure coordinated care.
• Maintain accurate clinical documentation in compliance with standards.

Talin Home Healthcare LLC is looking for multiple compassionate and dedicated Home Health Nurses to serve patients across Arlington, Alexandria and Fairfax area. Responsibilities

• Provide skilled nursing care to patients in their homes across the Northern Virginia area
• Conduct comprehensive patient assessments and develop individualized care plans
• Must have experience with administering medications, wound care wet and dry, wound vac, IV therapy, ostomy care and other clinical treatments as ordered.
• Monitor and document patient progress, vital signs, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Collaborate with physicians, therapists, and care team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements

• Active RN license in the state of Virginia
• Previous home health, hospital med-surg, or community nursing experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Reliable transportation and valid driver's license for travel throughout the NOVA region
• Ability to work independently and manage a flexible patient caseload
• Compassionate, patient-centered approach to care
• Must commit to working a minimum of 2 days a week 1 day of the days must be a Monday, and the other can be Wednesday or Friday.
• Immediate Opening to start asap.
