Job summary

Enhabit Home Health & Hospice is seeking a skilled RN care coordinator to ensure high-quality home health services and seamless patient coordination. You will review start-of-care documents, work with physicians and caregivers, and schedule field staff to maintain care continuity.

Ideal candidates have 2+ years of recent nursing experience (1+ in home health), a valid nursing license (compact accepted), CPR, and strong communication. Proficiency with MS Office and Homecare Homebase is a plus.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license per state requirements or compact license.
• Minimum two years of nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification and valid driver license with insurance.
• Proficiency with technology including mobile devices and MS Office.

Responsibilities

• Coordinate overall delivery of home health services according to standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication among physicians, referral sources, patients, caregivers and field staff to ensure care coordination and continuity.
