Job summary

Optum Home & Community Care, part of UnitedHealth Group, is expanding its HouseCalls program to bring preventive care directly to patients' homes. As a HouseCalls Advanced Practice Clinician (NP/PA), you will conduct annual in-home health assessments for Medicare Advantage and other plan members with flexible scheduling between 8am and 8pm, across a 7-day week.

The role offers work-life balance with 20–40 hours per week, no on-call, weekends, or holidays.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Gerial or Adult-Gerontology Primary Care.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other plan members.
• Document diagnoses and care plans; communicate findings to PCPs to address gaps in care.
• Educate members on diseases, medications and adherence.
• Identify urgent situations and intervene appropriately.
• Address social determinants of health and provide referrals as needed.
