Job summary

OptumCare is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members. You will conduct comprehensive reviews, document diagnoses, and coordinate with PCPs to close care gaps.

Flexible scheduling (20-40 hours/week) with no on-call, weekends, or holidays, and a supportive team environment. Eligible providers receive student loan repayment or sign-on bonus, plus a robust benefits package.

Qualifications

• Active ANCC or AANP certification in primary care
• Current NCCPA certification and state licensure for PAs
• Active, unrestricted licensure in the practicing state
• Access to reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home assessments including history, medications, vitals, and exams
• Identify and document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address gaps in care
• Recognize urgent/emergent situations and intervene appropriately
• Educate members on diseases, medications, and adherence
• Address social determinants of health and provide referrals as needed
