UnitedHealth Group's Optum Home & Community Care is hiring a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage and other members. The role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes, with field-based, non-prescribing duties.

Qualifications

• Active nursing license in the practing state
• Active ANCC or AANP certification as NP (Family/Adult/Geriatric)
• Experience with in-home assessments or home health is preferred
• Strong communication with geriatric/Medicare populations

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plan members
• Document diagnoses for care management and treatment planning
• Communicate findings to members' PCPs to address care gaps
• Educate members on disease processes, meds, and adherence
• Address social determinants of health and provide referrals as needed
