Job summary

Tennessee Home Health in Cleveland, TN seeks a Medical Social Worker (MSW) to support hospice patients and their families in the community. The MSW provides counseling, resource coordination, and care planning to address psychosocial needs with dignity and respect.

Responsibilities include assisting with financial, social, and emotional concerns; communicating changes to the care team; educating families on social work interventions; and coordinating transfers and discharges.

Qualifications

• Master's degree in Social Work (MSW) or Bachelor of Social Work (BSW) from CSWE-accredited program.
• Must hold a current, unencumbered Social Worker license in the state of service.
• Minimum 1 year of social work experience in a healthcare setting.
• Current CPR certification, valid driver's license, vehicle insurance, and access to reliable transportation.

• Perform care planning, transfer and discharge functions as part of the care team.
• Educate patients, families, and other team members on social work interventions and treatment plans.

POSITION OVERVIEW:

The Medical Social Worker provides emotional support, counseling, and resource coordination to patients facing terminal illness. This role plays a key part in the delivery of holistic care and ensures that patients' psychosocial needs are met with dignity and respect. A few position highlights include:

• Assist the patient and family with health related financial, social and emotional concerns and assist with effective coping skills.
• Provide social work services in accordance with the plan of care.
• Ensure that changes in the patient’s clinical status are communicated to other healthcare team members and the agency in a timely manner.
• Educate the patient and family members and other team members on medical social work interventions and treatment plan.
• Perform care planning, transfer and discharge functions.
• Identify risks or threats to the patient’s health in the patient’s environment and promptly implement appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient, the physician, and other members of the healthcare team to coordinate care effectively.

THE MUST HAVES:

• Master's degree (MSW) or (BSW) from a school of Social Work accredited by the Council on Social Work Education.
• Must have a current and unencumbered Social Worker License in the state where services are rendered.
• Must have one (1) year of social work experience in a health care setting.
• Current CPR certification, driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.
