Tennessee Quality Care Hospice is seeking a licensed PRN Social Worker to join our team in Martin. You will provide psychosocial counseling, resource coordination, and collaboration with the clinical team to support holistic home-based care for patients and families.

Requirements include a Master’s in Social Work and Tennessee licensure (LMSW or LCSW). This is an as-needed position with Monday–Friday 8:00am–4:30pm shifts, serving Martin, TN and surrounding counties.

Qualifikationen

• Minimum of one year’s experience in health care or hospice setting.
• Valid driver’s license and proof of insurance required.

• Performs psychosocial assessments of patients and families in the hospice program and integrates needs into the plan of care.
• Demonstrates exceptional service to patients, families, referral sources, co-workers and other customers.
• Counsels patients and families on stress and coping.
• Documents patient services accurately and timely to reflect the plan of care.
• Assists with applications for SSI, Hospice, Medicaid and Medicaid Waiver; coordinates resources from community agencies.
• Acts as liaison between patients, families and community agencies.

What You’ll Do:

• Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient’s plan of care. Provides an assessment in the patient’s identified residence and assistance when this is not safe, and another plan is required
• Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
• Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
• Provides accurate and timely documentation of patient services to reflect the plan of care
• Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
• Serves as liaison between patients and families/caregivers and community agencies.
