Home Care Solutions in Dickson, TN is seeking a Social Worker – Home Health to provide emotional support, counseling and resource coordination for patients facing terminal illness. The MSW/BSW-qualified candidate will travel to multiple patient residences daily within the assigned area.

The role emphasizes holistic care, plan-of-care adherence, and effective communication with the healthcare team to ensure dignified, patient-centered support.

Qualifications

• Master's degree (MSW) or (BSW) from a CSWE-accredited school of Social Work.
• Must have a current and unencumbered Social Worker License in the state where services are rendered.
• Must have one (1) year of social work experience in a health care setting.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Must be able to reliably commute to multiple patient residences daily within the geographic area assigned.

Responsibilities

• Assist the patient and family with health related financial, social and emotional concerns and assist with effective coping skills.
• Provide social work services in accordance with the plan of care.
• Ensure that changes in the patient's clinical status are communicated to other healthcare team members and the agency in a timely manner.
• Educate the patient and family members and other team members on medical social work interventions and treatment plan.
• Perform care planning, transfer and discharge functions.
• Identify risks or threats to the patient's health in the patient's environment and promptly implement appropriate interventions to reduce risks.
• Communicates identification of risks/ threats to the patient, the physician, and other members of the healthcare team to coordinate care effectively.

POSITION OVERVIEW:
