Job summary

Optimal Care in Carmel, IN is seeking a Home Health Medical Social Worker to provide psychosocial support, assessments, counseling, and resource coordination for patients and families in their homes and facilities.

You will collaborate with nurses, therapists, physicians, and caregivers to develop individualized care plans, address social determinants of health, and support patient independence across Boone, Clinton, Hamilton, and surrounding counties.

Qualifications

• MSW or LMSW/LCSW licensure in the state
• CSWE-accredited MSW degree
• Willingness to travel daily

Responsibilities

• Perform psychosocial assessments and counseling
• Coordinate resources and education for patients and families
• Develop individualized home care plans with the care team
• Travel daily to patient homes within service area
• Support patient and caregiver independence
