Job summary

Mercy Health Home Care and Hospice by Compassus is seeking a Hospice Social Worker to provide psychosocial assessments, counseling, and support within the interdisciplinary team. You will promote Compassus values and coordinate care planning across settings to optimize coping for patients and families.

The role emphasizes advance care planning, living will/POA facilitation, and linkage to community resources, while upholding ethical conduct and nondiscrimination as part of compassionate hospice

Qualifications

• MSW from a CSWE-accredited program with at least 1 year healthcare experience.
• State licensure as required in practice.
• Bachelor’s degree in social work with 1 year healthcare experience considered for MSW-supervised locations.

Responsibilities

• Explain hospice services and Medicare benefits to patients and families.
• Submit appropriate documentation to facilities after visits.
• Participate as a member of the IDT and develop the plan of care.
• Provide psycho-social support and counseling to patient and family.
• Identify and address needs with IDT to ensure quality of care.
• Maintain updated care plans reflecting problems, goals and interventions.
• Advance planning including living will/POA documents and related tasks.
