Job summary

Optimal Care in Carmel, IN seeks a licensed Hospice Medical Social Worker (LMSW or LCSW) to provide compassionate psychosocial care, assess needs, and support advance care planning. You will join an interdisciplinary team to ensure emotional, cultural, and practical needs are integrated into each patient’s plan of care.

The role requires travel to patients’ homes and facilities, Monday–Friday, with competitive pay and benefits designed to support professional growth and work-life balance.

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work.
• Current State License as a Master’s Social Worker (MSW/LMSW/LCSW).
• Minimum 1 year of experience as a licensed Master’s Social Worker.

Responsibilities

• Provide comprehensive psychosocial assessments.
• Offer crisis intervention and counseling.
• Support advance care planning conversations with patients and families.
• Collaborate with nurses, physicians, and other hospice professionals as part of the interdisciplinary team.
