Job summary
Tri County Home Health Care seeks a PRN Medical Social Worker (MSW) to deliver psychosocial assessments and coordinated home-based care for patients of all ages. You will analyze, counsel, and refer to meet patient and family needs while supporting overall care coordination.
Reports to the Director of Nursing and works in patients' homes, with travel as needed to provide compassionate social work services.
Qualifications
- Master's or doctoral degree in social work from an accredited program.
Responsibilities
- Provide coordinated home-based care to patients of all ages, including psychosocial assessments and referrals.
- Conduct analysis and counseling to meet patient and family needs and coordinate care.
