Job summary

Optimal Care is seeking a Home Health Medical Social Worker in Michigan to provide psychosocial support, education, and resource coordination for patients and families at home. You will collaborate with nurses, therapists, physicians, and caregivers to address emotional, social, and environmental needs within the care plan.

The role requires MSW licensure, strong counseling and communication skills, and daily travel within the service area to patients in homes and facilities.

Qualifications

• Master's or doctoral degree from a CSWE-accredited school of social work.
• Current State License as a Master's Social Worker (MSW/LMSW/LCSW).
• Minimum 1 year of experience as a licensed MSW.
• Reliable transportation with valid automobile insurance.
• Willingness to travel daily for home visits within service area.

Responsibilities

• Conduct psychosocial assessments and counseling to patients and families.
• Develop individualized care plans with nurses, therapists, and physicians.
• Coordinate community resources to support recovery and independence.
• Identify barriers to recovery and assist with discharge planning and care coordination.
