Job summary

OneWell Health Care is seeking a Licensed Practical Nurse (LPN) to provide skilled nursing services in home and community settings across South Carolina. The LPN will work under an RN, administer medications, monitor health, and support participants with IDD and other medical needs in a field-based role.

Qualifications include an active SC LPN license, CPR/BLS, and TB testing. Minimum 1 year of nursing experience is required; home health or IDD experience is preferred.

Qualifications

• Active South Carolina LPN license (required).
• CPR/BLS certification (required).
• Current TB test or screening (required).
• Minimum one year of nursing experience required.
• Home health/IDD/long-term care experience preferred.
• G-Tube/Trach/Vent care experience preferred (case dependent).

Responsibilities

• Deliver physician-directed nursing care per individualized care plans.
• Administer prescribed medications and treatments.
• Monitor vital signs and perform clinical tasks.
• Assist with PEG/G-Tube or tracheostomy care as trained.
• Maintain accurate documentation and reporting to RN and care team.
• Ensure compliance with state and agency regulations and care plans.
