Job summary
OneWell Health Care is seeking a Licensed Practical Nurse (LPN) to provide direct clinical nursing care in home and community-based settings across South Carolina. You will administer medications, monitor health status, and support individuals with IDD and other medical needs under RN supervision and physician orders.
The role requires SC LPN licensure, CPR/BLS, and the ability to travel to participant homes within the assigned region.
Qualifications
- Active South Carolina LPN license
- CPR/BLS certification
- Current TB test or screening
- Minimum one year of nursing experience
- Experience in home health, IDD services, or long-term care preferred
- Experience with G-Tube, Trach, or Vent care strongly preferred
- Ability to travel to participant homes within assigned region
Responsibilities
- Deliver physician-directed nursing care by implementing and monitoring individualized care plans
- Administer prescribed medications and treatments
- Monitor vital signs and provide wound care
- Support PEG/G-Tube or tracheostomy care as applicable
- Maintain accurate nursing notes and MAR
- Communicate changes in health status to supervising RN and families/caregivers
