Job summary

OneWell Health Care in Charleston, SC seeks a licensed Practical Nurse (LPN) for field-based home health services. The LPN delivers nursing care, administers medications, and monitors patient status under RN supervision across pediatric to geriatric populations.

Ideal candidates hold an active SC LPN license, CPR/BLS, and have at least 1 year of nursing experience. Travel within SC is required; flexible schedule offered.

Qualifications

• Active South Carolina LPN license required.
• CPR/BLS certification required.
• Current TB test or screening required.
• Minimum one year of nursing experience required.
• Experience in home health, IDD services, or long-term care preferred.

Responsibilities

• Deliver skilled nursing care under RN supervision.
• Administer medications and treatments per physician orders.
• Monitor health status and vital signs; respond to changes.
• Document nursing notes and MAR in EHRs daily.
• Support PEG/G-Tube or tracheostomy care as trained and needed.
