OneWell Health Care in South Carolina seeks a Licensed Practical Nurse (LPN) for field-based home health visits. The LPN delivers skilled nursing care under RN supervision, administers medications, monitors health status, and supports individuals with IDD in home and community settings.

Requirements include SC LPN license, CPR/BLS, TB screening, and at least 1 year of nursing experience; home health or IDD care preferred. Travel to participant homes within the region is expected.

Qualifications

• Active South Carolina LPN license required.
• CPR/BLS certification required.
• Current TB test or screening required.
• Minimum one year of nursing experience required.
• Home health/IDD/long-term care experience preferred.
• Experience with G-Tube, Trach, or Vent care preferred.

Responsabilités

• Deliver physician-directed nursing care by implementing care plans.
• Administer prescribed medications and treatments.
• Monitor vital signs and wound care.
• Support PEG/G-Tube or tracheostomy care as trained.
• Document promptly and accurately in EHR systems.
• Coordinate with families, caregivers, and providers; ensure compliance.
