Job summary

OneWell Health Care in South Carolina seeks a dedicated Licensed Practical Nurse (LPN) to join its home health team serving Daniel Island and surrounding communities. The LPN will deliver skilled nursing, medications, and wellness checks in participants’ homes.

The role requires an active SC LPN license, CPR/BLS certification, and a willingness to travel. This field-based position offers a flexible schedule and collaborative practice with RNs to promote safety and quality of life.

Qualifications

• Active South Carolina LPN license is required.
• CPR/BLS certification is required.
• Current TB test or screening is required.
• Additional certifications may be required depending on assignment.

Responsibilities

• Provide direct nursing care and administer prescribed medications.
• Monitor vital signs and perform basic wound care.
• Support PEG/G‑tube or tracheostomy care as trained and applicable.
• Maintain accurate documentation and communicate health changes to RN.
• Collaborate with families, caregivers, and healthcare providers to ensure quality care.
