Job summary

Louisiana HomeCare of River Parishes, part of LHC Group, seeks an LPN to provide quality patient care in home health settings. You will document visits, participate in care coordination, and work with the health care team according to the plan of care and infection control standards.

Required are current LA LPN licensure, a valid driver's license, and access to reliable transportation. Preferred: CPR, home care experience, and ability to multitask in a dynamic environment.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• LA: Current LPN Licensure in LA without restrictions.
• LA: A minimum, one year of experience working as an LPN.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and plan of care, following infection control procedures.
• Documents patient visits thoroughly and completely in the EMR and electronic visit verification program as applicable.
• Participates in care coordination, communicating with patient, family, physician, other clinicians, and case managers.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
