Job summary

Louisiana HomeCare of New Orleans, part of LHC Group, is hiring an LPN for Home Health to deliver quality patient care in patients' homes across Louisiana. Responsibilities include providing skilled nursing care per the plan of care, documenting visits in EMR, coordinating with the care team, and maintaining infection control standards.

Must have LA LPN license, valid driver’s license, and at least one year of experience in LA.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and access to a dependable vehicle or reliable transportation.
• LA-specific: LA LPN licensure with at least one year of experience.

Responsibilities

• Provides high quality clinical services within the scope defined by state laws, per the plan of care, and in coordination with the health care team.
• Documents patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participates in care coordination, reporting changes in condition and communicating with patients, families, physicians, and case managers.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
