Job summary

Louisiana HomeCare of River Parishes, part of the LHC Group within Optum family, seeks an LPN in Home Health to deliver quality patient care in patients' homes across LA. You will document visits, coordinate care with the health team, and adapt to diverse home environments while maintaining infection control and compliance.

Ideal candidates hold a current LA LPN license, a valid driver's license, and have strong communication skills to support patients and families.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• LA: Current LPN licensure in LA without restrictions.
• LA: A minimum of one year of experience as an LPN.

Responsibilities

• Provide high quality clinical services within scope of practice and plan of care.
• Document visits thoroughly in the electronic medical record per regulatory requirements.
• Participate in care coordination, report patient needs and changes in condition.
• Function professionally in diverse home settings and with all patients.
