Job summary

UnitedHealth Group, through Jeff Davis HomeCare, provides home health, hospice and community-based services in Louisiana. This LPN in Home Health role focuses on quality patient care within the home, following plan of care and infection control procedures.

Eligible candidates will have current LA LPN licensure, valid driver's license or reliable transportation, and one year of LPN experience. Strong communication and ability to manage multiple tasks are valued.

Qualifications

• Current and unrestricted LPN licensure in state of practice (LA)
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• LA State-specific requirement: Current LPN Licensure in LA without restrictions and minimum 1 year LPN experience
• Preferred CPR certification or ability to complete within 90 days of hire
• Home care experience recommended
• Ability to manage multiple tasks simultaneously
• Good communication, writing, and organizational skills

Responsibilities

• Provides high quality clinical services within the scope of practice and plan of care
• Documents patient visits thoroughly per regulatory and payer requirements in EMR
• Participates in care coordination, reports changes in patient condition
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition
