Zusammenfassung

Louisiana HomeCare, part of the LHC Group and within the Optum family, is hiring a Full Time Licensed Practical Nurse (LPN) for home health in Louisiana. You will provide skilled nursing care, document visits, and support care coordination for patients in their homes.

Required: current LA LPN license and at least 1 year in home care; valid driver's license or reliable transport; CPR preferred. Flexible hours and comprehensive benefits are offered to eligible employees.

Qualifikationen

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• LA: Current LPN licensure in LA without restrictions.
• Minimum one year of experience as an LPN.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Home care experience preferred.
• Ability to manage multiple tasks simultaneously.
• Able to work independently.
• Good communication, writing, and organizational skills.
• Ability to work flexible hours to meet client needs.

• Provide high quality clinical services within scope of practice per plan of care.
• Document patient visits thoroughly in electronic medical records.
• Participate in care coordination and report changes in patient condition.
• Function effectively in diverse home settings.
