Job summary

The Pines Home Health and Hospice in Prescott, AZ is seeking a dedicated Home Health LPN to provide compassionate patient care in the home setting. You will implement plans of care, document accurately, and collaborate with RNs and physicians to ensure seamless service.

Qualifications

• Current LPN license in the State of Arizona.
• Valid driver’s license and reliable, insured vehicle.
• Current Fingerprint Clearance Card or the ability to apply.
• Strong clinical skills and a passion for patient-centered care.
• Ability to work independently and make a real difference.

Responsibilities

• Implement plans of care initiated by the registered nurse.
• Provide accurate and timely documentation per the plan of care.
• Collect assessment data and report to RN(s) and physician(s).
• Coordinate home health services and report identified needs.
• Participate in personal and professional growth through training and mentorship.
