Job summary

Sacred Heart Home Health is seeking an LPN to provide hands-on patient care in home settings under RN supervision. You will ensure comfort, safety, and positive outcomes for patients while documenting progress and following care plans.

Ideal candidates have an AZ LPN license, CPR certification, and a preference for 3 years of nursing experience in home health or MedSurg. A valid driver’s license and reliable transportation are required.

Qualifications

• Graduate of an accredited practical/vocational nursing program.
• Current LPN/LVN license in the State of Arizona.
• Current CPR certification.
• 3 years of nursing experience preferred (Home Health or MedSurg a plus).
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Deliver compassionate, direct patient care in accordance with state regulations.
• Implement care plans developed by the RN and document progress accurately.
• Monitor patient conditions and communicate observations to the RN and physician.
• Collaborate with the care team to ensure seamless, high-quality service.
• Participate in professional development and team meetings.
