AHN Healthcare@Home in Apollo, PA is hiring a Home Health LPN to provide skilled nursing care in patients' homes within the territory. You will work with an interdisciplinary team, enjoy autonomy, and build meaningful patient relationships while growing your career.

This 40-hour per week employee role offers a $10,000 first-year success bonus, mileage reimbursement, and benefits including medical/dental/vision, PTO, 401K, and parental leave.

Qualifications

• Active LPN license in PA required.
• Two years of LPN experience.
• Valid driver’s license and ability to travel ~20% of workday to patient locations.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status
• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
• Maintain accurate, timely, and compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory

LPN Qualifications

• Active Licensed Practical Nurse (LPN) license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
