Job summary

The Pines Home Health and Hospice is seeking a licensed Physical Therapist to deliver in-home therapy in Arizona. You'll design patient‑specific care plans, provide hands‑on therapy, and coordinate with physicians to improve mobility and independence.

A flexible schedule supports work‑life balance and professional growth. Arizona licensure, CAPTE‑accredited education, and CPR are required; 2+ years' home health experience is preferred.

Requirements

• Licensed Physical Therapist in the State of Arizona
• Degree from a CAPTE-accredited program; passed state‑approved exam
• Minimum 2 years' experience (home health preferred)
• Current CPR certification
• Fingerprint Clearance Card (or immediate ability to apply)
• Valid driver’s license, insurance, and reliable transportation
