AHN Healthcare@Home is seeking a Licensed Practical Nurse to provide skilled nursing care to patients in their homes in the Johnstown, PA area. This home health role offers meaningful patient relationships, autonomy, and strong career growth opportunities.

A total bonus potential of $25,000 (including a $10,000 first year success bonus and a $15,000 sign-on) is available, contingent on performance and status.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience required.
• Current driver’s license and ability to drive ~20% of the day.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting as part of an interdisciplinary team.
• Perform comprehensive patient assessments including vital signs, medication management, and wound care.
• Educate patients and caregivers on disease management and prescribed treatments; ensure adherence and confidence in care routines.
• Maintain accurate EMR documentation and comply with all policies.

We are hiring a Licensed Practical Nurse for a Home Health position providing skilled nursing care to patients in their homes in the Johnstown, PAarea. This position is eligible for $25,000 total bonus potential, including a $10,000 first year success bonus to reward the accomplishment of key achievements during your first year as well as an additional $15,000 sign-on bonus paid within the first 18 months of employment! To receive these bonuses, you must be in good standing & meet performance expectations, and in the role and status for which you were hired!

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
• Educate and support patients and caregivers on disease management, medication administration, and prescribed treatments to encourage adherence and confidence in care routines
• Maintain accurate, timely, and compliant EMR documentation

• Work independently, but never alone
• Travel to patient homes within assigned territory

LPN Qualifications
