Job summary

Adoration Health in Westminster, MD seeks a licensed practical nurse to join our home health team. This full-time role provides 1:1 patient care in Carroll County, following each patient’s plan of care with dignity and independence.

You will administer medications and assessments, document nursing actions, and coordinate supplies while working with a supportive team. Prior home health and HCHB experience is preferred, with a compassionate approach to care.

Qualifications

• LPN/LPT/LVN license in good standing in state of practice and residence
• Valid driver’s license with proof of insurance
• CPR certification (professional)
• 1+ year nursing experience
• Home health care experience preferred

Responsibilities

• Provide 1:1 patient care according to plan of care while preserving patient dignity and independence
• Administer medications, feedings, oxygen, ostomy care as needed per plan and assess results
• Document nursing actions and communicate with family, case manager, and physicians; report changes in patient condition
• Coordinate or order supplies, medications, and feedings to ensure adequate stock
• Ensure patient needs align with physician instructions

Responsibilities

As a Home Health Licensed Practical Nurse, You will:

• Providing 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence
• Administering medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results
• Accurately documenting nursing actions of all care given and communication with family, case manager, and physicians as well as documenting the progress and outcomes for established goals and informing physician, case manager, and families of changes in client's medical condition and needs
• Coordinating or ordering supplies, medications, and feedings as needed to insure adequate amounts are always on hand
• Ensuring all patient needs are being met in accordance with the instructions of the physician

Qualifications

• LPN/LPT/LVN Degree from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence
• Valid driver’s license, acceptable driving record and proof of car insurance
• CPR professional certification
• 1+ years of nursing experience preferred
• Home H ealth care experience preferred

Salary Range

USD $38.00 - $44.00 / Hour
