Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health Licensed Practical Nurse (LPN) to deliver skilled nursing care in patients’ homes in Montgomery or Prince George’s County. The role emphasizes top-of-license practice, 1:1 patient relationships, and Epic EHR documentation.

It’s a full-time, on-site position with no on-call requirements and competitive benefits. Requires at least 1 year in acute care, Maryland LPN licensure, CPR, and passing the NAPNES or equivalent medication

Qualifications

• Minimum of one year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing.
• Current licensure in the State of Maryland.
• CPR certification required.
• Must have passed NAPNES or equivalent medication exam before administering medications.
• Must have a valid driver's license and reliable transportation to the work site.

Responsibilities

• Deliver skilled nursing care to clients per the plan of care prescribed by a physician.
• Practice at the top of your license with greater autonomy and meaningful 1:1 patient relationships.
• Document and coordinate care efficiently using the Epic EHR system.
