CenterWell Home Health is seeking an experienced Home Health LPN to provide one-on-one nursing care in patients’ homes. Reporting to the Clinical Manager, you will implement care plans, educate families, and maintain thorough documentation to support recovery and independence in daily living.
The role requires two years of LPN/LVN experience, a current nursing license in the state, CPR certification, and reliable transportation. A 40-hour week with a competitive benefits package is offered.
As a CenterWell Home Health nurse, reporting to the Clinical Manager, you will provide personalized, one‑on‑one care that helps patients regain independence in the comfort of their homes. Working with a dedicated team of physicians and clinicians, you’ll manage care plans that support recovery and help patients get back to the life they love.
TB Statement: This role is considered patient‑facing and is part of Humana’s Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB.
Driving Statement: This role is part of Humana’s driver safety program and therefore requires an individual to have a valid state driver’s license and maintain personal vehicle liability insurance in accordance with state minimum required limits, or $25,000 bodily injury per person/$25,000 bodily injury per event, $10,000 for property damage, or the higher amount.
