Job summary

Elite Home Health, part of the LHC Group and Optum family, seeks a referral coordinator to manage patient referrals to home health. You will act as a liaison among patients, physicians, and the agency, coordinating follow-up and ensuring orders are properly processed.

Responsibilities include evaluating patient suitability, routing referrals to the correct branch, and coordinating physician documentation for face-to-face encounters. Strong communication and CRM skills are essential.

Qualifications

• Formalized clinical training from an accredited institution.
• Licensed in the state of practice.
• Home Health experience is advantageous.
• Solid computer skills, including Microsoft Outlook and CRM software.

Responsibilities

• Coordinates the referral of patients to home health, ensuring patient choice is honored.
• Evaluates patient's suitability for home health upon physician/facility request.
• Receives referrals and routes to the appropriate branch.
• Acts as a liaison between patient, physician, and agency.
• Schedules physician follow-up appointments when appropriate.
• Communicates reports between field staff and physician/nurse.
• Follows up to ensure orders reach the agency and are signed.
• Facilitates obtaining physician documentation for face-to-face encounters.
