Job summary

Enhabit Home Health & Hospice seeks a dedicated professional to coordinate patient referrals and support the transition to services. You will work with branch and sales teams, applying knowledge of regulatory guidelines to ensure compliant, timely care.

Ideal candidates have at least two years in health care, one year in home health or pediatrics, a valid nursing license (RN or LPN) preferred, strong customer service skills, and basic technology proficiency including mobile devices.

Qualifications

• Must possess a high school diploma or equivalent.
• Must have at least two years of experience in the health care industry and one year experience in home health or pediatrics.
• Must have demonstrated experience in customer service.
• Must have demonstrated knowledge of Medicare guidelines that govern a home health or hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure adequate coordination and transition to services.
