Job summary

Enhabit Home Health & Hospice is seeking an intake/referral coordinator to support patient referrals and transitions to services. You will work with the branch and sales teams to coordinate care, ensure compliance with Medicare guidelines, and help maintain smooth patient journeys in home health settings.

The role requires a high school diploma, 2+ years in healthcare with home health or pediatrics exposure, and solid customer service and tech skills.

Qualifications

• High school diploma or equivalent.
• At least 2 years in healthcare and 1 year in home health or pediatrics.
• Demonstrated customer service experience.
• Knowledge of Medicare guidelines for home health or hospice.
• Basic technology skills and mobile device operation.

Responsibilities

• Coordinate patient referrals and serve as a resource for referral sources.
• Collaborate with the branch and sales teams to ensure coordination and transition to services.
• Apply and demonstrate knowledge of regulatory guidelines and governing processes.
