Qualifications

• High school diploma or equivalent; associate or bachelor's degree preferred.
• Healthcare experience in intake, scheduling, or home health preferred.
• Strong organizational and time-management skills with attention to detail.
• Excellent written and verbal communication abilities.
• Ability to work independently as well as collaboratively with a team.
• Proficient in Microsoft Office Suite and EHR/EMR systems.
• HIPAA compliance awareness.

Responsibilities

• Process and review incoming patient referrals to ensure timely entry into the system.
• Coordinate with scheduling and clinical teams to ensure initial visits are scheduled promptly.
• Act as primary contact for patients, families, and referral sources, guiding them through onboarding.
• Provide administrative support to the sales team, including managing referral documentation and tracking statuses.
• Enter patient referral and intake information into the agency’s EHR system.
• Regularly audit ongoing referrals and patient files for accuracy and completeness.
• Prepare regular reports on referral trends and onboarding metrics for the team.
• Assist with special projects as assigned by the Director of Intake and Sales Support.
