Job summary

Talented Medical Solutions is seeking a skilled Referral Coordinator to support home health services in California. You will manage incoming referrals, coordinate with patients, clinicians, physician offices, and community liaisons, and ensure timely communication across teams.

The role requires an Associate degree nursing or equivalent, active RN/LVN/LPN license in the state, and EPIC experience, with at least 1.5 years of relevant experience.

Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m., 5×8-hour shifts.
Contract: November 9, 2026 – February 7, 2027
