Westminster-Canterbury on Chesapeake Bay, a not-for-profit life plan community, is seeking a Registered Nurse (RN) Case Manager for Hospice. This on-site, full-time role involves patient assessments, care planning, education for patients and families, and collaboration with the interdisciplinary hospice team.
The RN will supervise LPNs and Hospice Aides, ensure compliance, participate in performance improvement, and rotate on-call.

The Registered Nurse (RN) Case Manager provides comprehensive nursing care to hospice patients by conducting initial and ongoing assessments of the impact of a terminal diagnosis on the patient's physical, functional, psychosocial, and environmental needs, as well as activities of daily living. This role develops, implements, and revises individualized plans of care, provides ongoing education and support to patients and their families, and collaborates closely with the interdisciplinary hospice team to ensure compassionate, high-quality care. The RN Case Manager also supervises Licensed Practical Nurses (LPNs) and Hospice Aides while consistently modeling Westminster Canterbury's core values of respect, passion, and professionalism and holding others accountable to the same standards.
