Job summary

VitalCaring delivers in-home occupational therapy services, seeking a Certified Occupational Therapy Assistant (COTA) to support one-on-one, goal-driven care. You will work under a licensed OT to help patients regain independence after illness or injury in their own homes.

This role emphasizes autonomy with team support, focusing on functional outcomes and patient safety while collaborating with nurses and leaders to deliver coordinated care.

Qualifications

• Graduate of an accredited Occupational Therapy Assistant program.
• Active COTA license/certification in the state of practice.
• Home Health experience strongly preferred.
• Current CPR certification.

Responsibilities

• Implement treatment plans to improve patients’ ability to perform daily activities.
• Provide hands-on therapy focusing on strength, coordination, mobility, and self-care.
• Retrain patients in ADLs such as dressing, bathing, and meal prep.
• Assess and report on patient progress, function, and home safety needs.
• Educate patients and caregivers on techniques, exercises, and safety strategies.
• Identify changes in condition and communicate with the OT and care team.
• Complete timely, accurate documentation in the EMR.
• Support a safe, effective, and coordinated plan of care.
