VitalCaring is seeking a Certified Occupational Therapy Assistant (COTA) for home-based, one-on-one patient care. You will work under an OT supervisor to help patients regain independence in daily living after illness or injury, in a field-based setting.

The role emphasizes autonomous practice with guidance from a seasoned OT team, a focus on functional outcomes, and collaboration with nurses and leadership to ensure quality care at home.

Qualifikationen

• Graduate of an accredited OTA program.
• Active COTA license/certification in the state of practice.
• Home Health experience strongly preferred.
• Current CPR certification.

Aufgaben

• Deliver personalized, goal-driven care under the supervision of a Licensed Occupational Therapist.
• Implement treatment plans to improve ADLs and daily living activities.
• Provide hands-on therapy focusing on strength, coordination, mobility, and self-care.
• Retrain patients in ADLs such as dressing, bathing, and meal prep.
• Assess and report patient progress, function, and home safety needs.
• Educate patients and caregivers on exercises, techniques, and safety strategies.
• Identify changes in condition and communicate proactively with OT and care team.
• Complete timely, accurate documentation in the EMR.
• Support a safe, effective, and coordinated plan of care.
