Job summary

Suburban Home Health in central Indiana seeks a dedicated Occupational Therapy Assistant (COTA) to support adult and geriatric patients in the home setting. You will work with the interdisciplinary team to execute the physician's plan of care and help restore function and daily activities.

You will educate patients and caregivers, document progress in the EMR, and participate in care coordination to ensure safe, effective therapy in the home environment.

Job Description

• The Occupational Therapy Assistant (COTA) is responsible for providing direct patient care to adult and geriatric patients in the home setting.
• Collaborates with the interdisciplinary team to implement a comprehensive plan of care with the physician
• Provides therapeutic care to help restore function and daily activities of living under the direction of the occupational therapist (OTR) in accordance with the physicians plan of care
• Participates in the education and delivery of the exercise program developed by the OTR for strengthening and controlling of the upper extremities
• Instructs patients and caregivers regarding plan of treatment, basic living skills and proper use of adaptive equipment
• Observes and documents patients progress and health status in a timely manner via electronic medical record (EMR)
• Provides teaching techniques, such as, the proper method of moving from a bed into a wheelchair, and the best ways to stretch and limber muscles to improve patients quality of life
• Participates in weekly case conference ensure effective coordination of care as well as monthly therapy in-services
• Assesses patient environment and initiates actions to prevent accidents and promote safety in the home setting
