Job summary

Qualifications

• Requires Bachelor's Degree in healthcare administration, marketing, or related field; or nursing/therapy/social work graduate.
• Minimum two years in healthcare business development, community relations, marketing, or sales.
• Valid state driver license and automobile liability insurance.
• Knowledge of federal, state, and local laws governing Medicare home health/hospice operations.
• Strong interpersonal, communication and presentation skills.

Responsibilities

• Acts as primary relationship manager with referral sources (physicians, hospitals, health systems, senior living communities).
• Promotes awareness of services and program requirements to referral sources, patients, families, and care partners.
• Supports referral growth and quality patient transitions to improve outcomes and satisfaction.
